AMENDMENT TO COMBINED SYNOPSIS/SOLICITATION
This amendment is issued to provide responses to a vendor inquiry. The Government received an inquiry regarding the onsite shredding services for the Yakama Service Unit, located in Toppenish, WA.
Acknowledgment Requirement: Respondents must confirm in their quote submission that they have reviewed and will comply with all information, requirements, and conditions provided in this amendment. Failure to acknowledge this amendment may result in the quote being deemed non-responsive.

DESCRIPTION
Title: Yakama Indian Health, Onsite Shredding Services
This requirement is for one (1) Firm-Fixed-Price (FFP) purchase order consisting of a one-year base period and four (4) one-year option periods for the lease of locked shred bins and secure onsite shredding services for the destruction of sensitive documents no longer required by the Service Unit[SS1] . Contractor shall comply with all applicable Indian Health Manual (IHM) requirements, records management policies, and NSA/CSS standards related to the safeguarding and destruction of sensitive documents records retention and destruction requirements.

1. Scope of Work
Introduction: The Portland Area Indian Health Service (IHS) provides healthcare services to approximately 150,000 American Indian and Alaska Native residents throughout Oregon, Washington, and Idaho. The Portland Area IHS operates four federal health facilities located within three tribal communities and one facility at the Chemawa Indian School.
The Yakama Indian Health Center is located in Toppenish, Washington, on the Yakama Indian Reservation. The Yakama Service Unit requires a contractor to provide locked shred consoles and secure document destruction services. The services shall be performed on a recurring monthly basis, with additional one-time shredding services provided on as-needed basis.
The contractor is not required to access the interior of the facility. Facility personnel collect and stage the locked shred consoles in a designated secure waste holding area located behind the facility. Once staged, the contractor is dispatched to collect and shred the materials onsite. This process ensures sensitive materials are handled securely and efficiently while minimizing disruption to daily facility operations.
1.1 General Requirements
The Yakama Service Unit requires the lease of locked shred bins and consoles, as well as the provision of secure onsite shredding services, to properly purge and destroy sensitive documents and materials no longer required by the Agency.
1.2 Specific Tasks
The Contractor shall provide all labor, supervision, personnel, materials, equipment, tools, supplies, transportation, and incidental services necessary to perform secure document destruction services and meet all performance standards and compliance requirements under this contract. The shredding process shall ensure the confidentiality, security, and integrity of all materials collected and destroyed in accordance with applicable federal standards and industry best practices. All document destruction shall be performed in a manner that prevents unauthorized access, reconstruction, or disclosure of information.
The Contractor shall provide the following services:
- Supply locked shred consoles and containers for recurring monthly onsite shredding and disposal services.
- Provide on-demand, one-time shredding services upon Government request in addition to recurring monthly services.
- Ensure all shredding services meet or exceed cross-cut shredding standards recommended by the National Security Agency (NSA)/Central Security Service (CSS).
- Provide service reports, manifests, and/or Certificates of Destruction upon request by the Government.
- Notify the Facility Manager at least forty-eight (48) hours in advance of any scheduled service, schedule change, or anticipated service interruption.
Container Lease Requirements
The Contractor shall provide the following locked collection containers:
- Eight (8) 64-gallon wheeled bins with document insertion slots
- Three (3) 32-gallon wheeled bins with document insertion slots
- Two (2) locking consoles
All bins and consoles shall:
- Be lockable and securely constructed
- Be keyed alike
- Include one (1) key provided to the Government for each bin and console
Shredding Specifications
- Paper shall be shredded to particles measuring 1 millimeter by 5 millimeters or smaller (cross-cut).
- Shredding services shall include all paper-based media and similar materials.
- All shredding shall be performed onsite at the Government facility unless otherwise authorized by the Contracting Officer.
3. Security Requirements
Contractor personnel shall have intermittent access to Government property only as necessary to perform required services. Contractor personnel shall be escorted by Government personnel while onsite.
No background investigation is required for Contractor personnel under this requirement.

6. Other Pertinent Information or Special Considerations
The Contractor shall comply with all applicable federal, state, local, and tribal laws, regulations, standards, and ordinances.
All destruction methods and equipment shall comply with applicable requirements identified in NSA/CSS Policy Manual 9-12, as applicable.
All services shall be performed in a manner that maintains the confidentiality, integrity, and security of sensitive materials at all times and prevents unauthorized access, disclosure, or reconstruction of information.

8. Period of Performance
The period of performance shall consist of a one (1) year base period and four (4) one-year option periods. Performance shall begin on the date of contract award or as otherwise specified in the Notice to Proceed.
9. Performance Requirements
The Contractor shall perform all services in accordance with the requirements of this Performance Work Statement (PWS) and maintain a level of quality that ensures secure, timely, and reliable document destruction services.
9.1 Service Performance Standards
The Contractor shall meet the following performance standards:
- Perform all recurring shredding services on scheduled service dates and times.
- Respond to Government requests for on-demand shredding services within the agreed response timeframe.
- Maintain the security and confidentiality of all materials collected, transported, handled, and destroyed.
- Ensure all shredded materials meet cross-cut particle size requirements of 1 millimeter by 5 millimeters or smaller.
- Provide Certificates of Destruction, manifests, or service reports upon request by the Government.
- Maintain all leased containers and consoles in functional, lockable, and serviceable condition.
- Provide at least forty-eight (48) hours advance notice of any scheduled service interruption, delay, or schedule change.
- Perform all services in compliance with applicable federal, state, local, tribal, and NSA/CSS requirements and standards.
- Ensure contractor personnel comply with all Government facility access, escort, and security requirements while onsite.
- Perform services in a manner that minimizes disruption to Government operations and protects sensitive information from unauthorized access, disclosure, or reconstruction.
9.2 Contractor Responsibilities
The Contractor shall:
- Maintain adequate staffing and equipment necessary to perform required services.
- Ensure all contractor personnel comply with facility access and escort requirements.
- Replace damaged or nonfunctional containers within a reasonable timeframe upon notification by the Government.
- Perform all services in a professional and safe manner that minimizes disruption to Government operations.
9.3 Quality Assurance
The Government will monitor Contractor performance through:
- Visual inspection of services performed
- Review of service documentation and Certificates of Destruction
- Validation of schedule compliance
- Customer feedback and complaint tracking
The Contracting Officer Representative (COR) or designated Government representative will document deficiencies and coordinate corrective actions with the Contractor as necessary.

Factor 1 – Demonstrated Technical Capabilities
Technical capabilities will be evaluated based on the Offeror’s demonstrated ability to:
- Provide secure onsite shredding services
- Provide and maintain required locked shred bins and consoles
- Meet required shredding specifications and security standards
- Perform recurring and on-demand services in accordance with the PWS
- Provide documentation such as manifests and Certificates of Destruction
Factor 2 - Price
Price will be evaluated for completeness and reasonableness. The offeror shall provide all-inclusive pricing, including equipment, delivery, installation, removal, travel, labor, and any other associated costs.
